Diabetes Community Services
Our Diabetes Self-Management Education Program is nationally recognized by the American Diabetes Association. The program provides prevention education and management services through group and individual sessions by Certified Diabetes Educators. Community involvement includes, annual Diabetes Info Fair, supports groups in diabetes screenings, and a partnership with school nurses in the "Prevention of Diabetes in Children". We also provide the following services and functions:

Diabetes Program Receives Large Grant from Kaiser
DMC Foundation’s Diabetes Community Services program was recently awarded nearly $50,000 from Kaiser Permanente. This generous grant is to be used in diabetes prevention education and assessment of adolescents at the Junior High school level.
